If the plaster has blown from the background this could be from either the background was too dry, dusty or lack of plaster primer when the plaster was applied.

Plaster should be removed and check the surrounding area by knocking on the wall and listening for the hollow sound.

Prime the background after removing blown plaster and reapply new plaster and make good.
